About the practice

The surgery is situated in Tuxford a couple of minutes from the A1 junction which provides an amenable travel corridor from Newark to Doncaster and beyond.

The practice provides unrivalled access for their 5,200 patients and have a rapidly increasing list size. They are a partial dispensing practice dispensing to about 50% of their patients.

Their GPs are supported by two ANPs (who contribute to triage and Face to Face appointments within their competencies), three Practice Nurses, two HCAs and a Pharmacist. The reception, admin and dispensary teams are dedicated to supporting the clinical workforce to ensure they have a smooth running practice.

They are an innovative practice who are continually looking to improve the way they operate to provide the highest level of timely care to their patients. They use EMIS Web Clinical System, but training will be provided to those not familiar with this system.

Job summary

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Clinical responsibilities

• In accordance with the practice rota, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
• Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
• Assessing the health care needs of patients with undifferentiated and undiagnosed problems
• Screening patients for disease risk factors and early signs of illness
• Developing care plans for health in consultation with patients and in line with current practice disease management protocols
